Blue Cross Blue Shield of MN: -Single Plan - Free premium. $3,000 deductible/$4,000 oop max -Family Plan - $298.63 per pay period. Deductible of $6,900 per member or $8,000 per family. -HSA contribution for all staff that participate in the medical. FM puts in $1,500/Year paid out monthly. -Employer Paid Life of $25,000 for all staff working 30+Hours. -Additional voluntary Life/AD&D for all staff working 20+ hours per week. -Voluntary Paid Short Term Disability for all staff working 20+ hours per week. -Voluntary Life for all staff working 20+ hours per week. -Voluntary Accident for all staff working 20+ hours per week. -Voluntary Critical illness for all staff working 20+ hours per week. -Free EAP Dental Insurance premiums per pay period: Eligible if you work at least 16 hrs per week. Pay Day: Bi-weekly every other Friday. Wed thru Tuesday is our pay period. Vacation: For non-flex staff - vacation hours can be used after 30 days of employment. They accrue day one. Sick Leave: For non-flex staff - sick leave can be used after 30 days of employment at Fair Meadow. They accrue day one.
